Liability waiver
— I release KISLAP Football Academy, its coaches, and staff from any liability for injuries or losses my child may sustain during the trial, training, matches, or related activities, and I confirm my child is physically fit to participate.

Cancellation policy
— I understand I can only cancel or reschedule up to 9 PM the day before the trial. If I cancel after that or don't show up, a free trial is forfeited and my next trial must be the paid trial (₱300) — and a paid trial fee is forfeited, so rebooking means paying ₱300 again.
